Rates
Insurance Clients:
Phoenix Behavior Services, LLC accepts Medicaid Home and Community Waiver (MedWaiver), the Family
Supporting Living Waiver (FSL Waiver or "Little Waiver"), and the Familial Dysautonomia Waiver Insurance. Please
contact your support coordinator for a referral to behavior analysis services. We do not accept direct referrals from
clients--your support coordinator must initiate the referral process.
Please note that we do not accept standard Medicaid.
We accept Magellan for autism/ABA clients who meet the medical necessity criteria, and for whom ABA services are authorized.
Please note the Magellan criteria is as follows: "Magellan considers applied behavioral analysis (ABA) investigational
and not medically necessary for individuals with autism spectrum disorders. However, Magellan
covers ABA when state-mandated or when ABA is specfically included in a customer's benefit plan." Please contact Magellan
to determine whether your case meets criteria BEFORE initiating services.
Other Insurances / Non-Insurance Clients:
Rates for behavior analysis services range from $65 to $110 per hour, depending on the behavior analyst used.
We will provide you with an invoice, which you may submit to your insurance company for reimbursement. However,
you will be responsible for paying for services, regardless of whether your insurance company choses to reimburse you.

The number of hours per week necessary for effective services will depend on the needs of the client, the type of behavior,
and its severity. Frequency of meetings ranges from 1-2 hours/week to 40 hours/week. Your behavior analyst will discuss
with you the appropriate number of hours of service per week during your initial session.
